FXO (voice-port) showing OFF-HOOK even after disconnect call

Hi All,
 We have UC520 with 4 FXO & two analog line, My scenario is when anyone made calls on 08 9377 3097 (voice-port0/1/1) means All incoming call which are coming  on 08 9377 3097 (voice-port0/1/1) should be diverted to 206 (ext.). If anyone are unable to receive call on 206 within 15 sec. then call divert to my  Mobile from 08 9377 0540 (voice-port0/1/0)
After disconnecting Incoming call means Hang up the phone & make call again on same no. Voice-port 0/1/1 that time I found  08 93773097 (Voice-port 0/1/1 ) is still engaged(busy) & also saw voice port is going to OFF-HOOK.
I have use below configuration :voice class custom-cptone test  dualtone disconnect   frequency 476   cadence 488 488 488 488 487 487 487 487 ! voice-port 0/1/0  supervisory custom-cptone test  no battery-reversal  output attenuation -6  cptone AU  timeouts call-disconnect 3  timeouts wait-release 3  connection plar opx 202  impedance complex1  caller-id enable ! voice-port 0/1/1  supervisory custom-cptone test  no battery-reversal  output attenuation -6  cptone AU  timeouts call-disconnect 3  timeouts wait-release 3  connection plar opx 206  impedance complex1  caller-id enable ! ephone-dn  15  dual-line  number 206 no-reg primary  label 206  description Lindsay Clark  name Lindsay Clark  call-forward busy 0862101169  call-forward noan 0862101169 timeout 10
 Can anyone help me on this Issue ?

Try with supervisory disconnect dualtone mid under fxo port.

maybe you already found the solution, but I got the exact same problem, and after trying everything zvuksanovic advice was the solution.
My voice port configuration :
